Sports Medicine Profile includes:

​1. SportsWare (SWOL) completed/updated
​2. Update yearly physical upload to SWOL
3. Upload any injury or illness release from doctor or PT to SWOL
​4. NCAA Medical Exemption forms if taking banned medicine for ADHD or breathing issues
​5. Sickle cell trait test results (freshman and transfers only) upload to SWOL
6. Concussion Testing on campus (freshman and transfers only) - download the SWAY Medical app for Android or iPhone
 
1. SportsWareOnline (SWOL) entry portal is closed until end of May
This is an online collection point for all sports medicine paperwork at www.swol123.net
Student-athletes must complete this every year- not your parents

Directions for SWOL access: First-Year/Transfer Instructions (Returners will use the same email as they did with Healthy Roster). In SWOL, you create an account, complete the assigned information, and sign forms (which require electronic signatures).

2. Yearly Physical: now available
Muskingum history/physical form is located in SWOL and on this website HERE. Download and take it with you to your doctor's appointment. Student athletes are responsible for scheduling and payment of their own physicals. We accept MD, DO, PA and NP but no chiropractic physicals.

3. Injury/Illness Releases:
Please remember if you saw a medical practitioner (MD, DO, NP or PA) for any injury, illness, PT or surgery within the past year you must have your medical release for participation. Upload it to SWOL or fax to (740) 826-6123.
 
HOW TO UPLOAD to SWOL: You will upload your completed history/physical, and other medical forms to SportsWareOnline (SWOL). This includes sickle cell results for freshmen/transfers only. Go to FORMS then use the +ADD button to upload. HEIC photos from iPhone are not recognized, convert to JPEG, PNG or PDF.

4. Medical Exemption Forms
This single form is for those taking medications for ADHD and/or breathing issues possibly banned by the NCAA. The NCAA Medical Exemption form for ADHD/breathing issues medications is located HERE and in SWOL under NOTICES AND HANDBOOKS. You only need to download and fill out this form if you are taking medication for these issues. Your prescribing practitioner must fill these forms out with you. Please upload the completed form and justification documentation to SWOL with your history/physical and releases. If you are taking a banned medication for other issues, please contact asmith@muskingum.edu for further information.

5. Sickle Cell Testing & Information
As of Aug 1, 2022, per NCAA mandate, ALL first-year/transfer student-athletes must show proof of a sickle cell trait test. This is requested just once. How do I find my results? Go HERE. What do I do with it once I have it? Upload it to SWOL with your history/physical and releases.
 
6. Concussion Testing
We will conduct SWAY concussion testing when student athletes arrive on campus.
Fall First year/Transfer student-athletes will test during fall preseason.
